request form and the full terms and conditions. COMPLETE the refund request form with your name, address, actual

4. SEND, via first-class USPS mail within 60 days of date of purchase, the completed refund request form, original
UPC
and original dated receipt (with purchase price circled) to the address listed on the refund request form.
